Advocacy Award Nominations

The purpose of this award is to recognize outstanding contributions to provincial advancement that would impact quality of life for children and youth with disabilities and their families.

Eligibility:
Individuals; families; networks; organizations

Presentation:
The presentation of the Advocacy Award will take place at the Tribute Awards Luncheon during the Annual OACRS Conference.

Nomination Process:
Nominations will accepted from OACRS or its members.
Completion of a formal application is required.

Deadline:
June 30, 2012

Selection Process:
Applications will be reviewed by the Board Development Committee. Recipients will be selected by the Board of Directors. Only one recipient will be selected in a given year. Other nominees will receive an honourable mention.

Application Requirements:
  1. Support Documentation for Nomination:
    • In an essay of no more than 350 words, describe how, Please describe how, through significant achievements or contributions on a provincial level, this nominee has impacted the quality of life for children and youth with disabilities and their families.
    • A letter of recommendation from a CTC must be included in the application
    • Additional letters of recommendation from community partners, families, etc, may be included.
    • A list of CTCs who have benefitted must be included in the application
  2. Please indicate whether or not the Nominee is aware of the nomination and in agreement to have their name and organization put forward for consideration by OACRS for a Partnership Award?
  3. Please indicate whether of not the Nominee is available to attend the Tribute Luncheon on Monday, 5 November 2012?
OACRS may request additional information to support the nomination.
